Medical Information Notice

Not a Medical Provider

Element NEMT is a non-emergency medical transportation (NEMT) company. We are not a medical provider, healthcare facility, or emergency medical service. Our drivers are not licensed to provide medical care, diagnosis, treatment, or clinical assessment during transportation.

Health Information We May Receive

When you request transportation, you may voluntarily share health-related information such as:

• ●Mobility level and mobility aid requirements
• ●Type of medical appointment or treatment destination
• ●Relevant access concerns (stairs, elevators, ramps)
• ●Special care notes provided by the client or caregiver

Purpose and Use

Any health-related information you provide is used solely to ensure that we can provide safe, appropriate, and properly equipped transportation. We use this information to assign suitable vehicles, prepare drivers for passenger needs, and coordinate with facilities when necessary.

Confidentiality

We treat health-related information with care and discretion. This information is shared only with drivers and operational staff as necessary to safely complete your transportation. We do not share health information with third parties for marketing or unrelated purposes.

Medical Emergencies

If a passenger experiences a medical emergency during transport, our driver will contact 911 immediately. Element NEMT drivers are trained in CPR and First Aid. However, they are not equipped or authorized to provide emergency medical treatment beyond basic first aid response. The health and safety of passengers always takes priority.

Health Information / HIPAA Notice

Element NEMT respects the confidentiality of passenger information and follows applicable privacy and HIPAA-related requirements when handling protected or sensitive information. Element NEMT does not provide medical treatment and does not store medical records or detailed health history. Any health-related information collected is limited to what is necessary to safely schedule, coordinate, and provide non-emergency transportation. Examples may include wheelchair use, mobility limitations, assistance needs, pickup and destination information, appointment location, or facility coordination details. Element NEMT does not sell or share health-related information for marketing purposes. Information is only shared when necessary for transportation coordination, billing, safety, legal compliance, or when authorized by the rider or designated responsible party.
