Element NEMT may receive limited health-related or mobility-related information when necessary to safely coordinate non-emergency transportation. This may include mobility status, wheelchair use, assistance needs, pickup and destination details, appointment location, facility information, or other transportation-related details. Element NEMT does not provide medical diagnosis, medical treatment, medical advice, emergency care, or ambulance-level care. Health-related information should be limited to what is necessary to safely and appropriately coordinate transportation.

We take reasonable measures to protect the personal information you provide. However, no method of transmission over the internet or electronic storage is completely secure.
